Leave a 2½-inch top margin on the first page and 1-inch margins on the remaining sides (Utah Code 17-21-20).

Pages must be 8½" x 11", black ink on white paper, legible enough to reproduce.

Include the document title, the names of all parties to be indexed, and the return-to name and address.

Conveyances must state the legal description and the tax parcel number, and give the grantee's mailing address.

Original signatures must carry a complete Utah notarial acknowledgment.

Documents affecting property in more than one county must be recorded in each county separately.
